What do we do?

As independent community health workers, we bring our training and experience to supporting our clients in finding success in the community and helping them overcome barriers along the way. Our services include, but are not limited to the following:

Explore a healthier and more fulfilling life with assistance in discovering health and wellness activities tailored to your needs.

Hello everyone. I am probably one of the most untrusting people on the planet. I have a hard time trusting anyone. I want to say first and foremost that Clyde is one of the most compassionate people I have ever met. He kept at me and never gave up. It was through him that Tiffany Harmon came into my life and began to help me get back onto my feet again. Tiffany helped me through probably the most difficult time in my life. I had lost my job, and time was not on my side. I was facing life threatening health issues, and I was on an eviction notice. Things weren't looking very good for me. Tiffany helped me get the help I needed and saw me through every step of the way. She is compassionate, dedicated, non-judgmental, generous and above all else... She is my friend. Who are we?

ln just a short time the Community health network has been able to supply food, housing, transportation assistance, and primary care support to a community that has been marginalized and look over for far too long.

We have also advocated for clients who have struggled in the legal system in and out of prison due to the inability to pay court fines which result into warrants & Probation violations.

Community health workers also work together with primary care physicians, mental health professionals, and other case workers to help create a care strategy, one that’s best fit for the client.
